# Important Information to Include in Your Report

Make sure your report includes the following details to help speed up the resolution:

**Location Details**: Province, District, Circuit, EMIS number, School Name
 **People Involved**: Name of Principal & Assistant
 **Contact Info**: Phone numbers of school, principal & assistant
 **PYEI Phase**: State that you are part of **Phase V****Nature of the Issue**: Describe the challenge you’re facing
 **Desired Solution**: What outcome you’re hoping for
 **Date of Submission**: When you’re sending the report
 **Signature**: Sign off as the Assistant
